The use of electroconvulsive therapy and general anaesthesia in catatonic schizophrenia complicated by clozapine - induced pancytopenia - case report.

چکیده
Catatonic schizophrenia is characterized with at least two of the following: catalepsy or stupor, excessive motor activity; extreme negativism or mutism; peculiarities of voluntary movement as evidenced by posturing, stereotyped movements, prominent mannerisms, or prominent grimacing; echolalia or echopraxia (American Psychiatric Association 1994). Electroconvulsive therapy (ECT) is widely used and shown to be effective and safe in patients with catatonic schizophrenia (American Psychiatric Association 2001). However, to our knowledge no case of ECT in catatonic schizophrenia complicated with pancytopenia due to clozapine sensitisation has been reported. Pancytopenia is defined as an disorder in which all three major formed elements of blood (red blood cells, white blood cells and platelets) are decreased in number (Istiaq et al. 2004).

Augmentative Asenapine in a Recurrent Manic Catatonic Patient with Partial Response to Clozapine
Catatonia is a severe but treatable neuropsychiatric syndrome known since the middle of the nineteenth century. It has been considered for a long time as a subtype of schizophrenia, even though this association occurs only in 10% of cases. In contrast, it is frequently observed in bipolar patients. First-line treatment consists of benzodiazepines, while in case of resistance electroconvulsive t...

Electroconvulsive therapy is a widely practiced, safe and effective treatment for severe mental illness. A febrile reaction induced by electroconvulsive therapy is rarely reported as an adverse effect. We present a 15-year old Chinese girl with first episode catatonic schizophrenia who developed recurrent febrile reactions after electroconvulsive therapy. Vigilant clinical monitoring and labora...
متن کاملSecondary enuresis & body dysmorphic disorder in a caucasian male with catatonic schizophrenia: a case report.
We describe a patient with Schizophrenia and secondary enuresis. The enuresis settled with resolution of his psychotic symptoms but later remerged after starting Clozapine. We explore the mechanisms of incontinence in Schizophrenia and those due to Clozapine. This case highlights the need to inquire about incontinence in patients with schizophrenia prior to prescribing clozapine.

Recently, as adjunctive treatment with clozapine, electroconvulsive therapy has been reported to be an effective and safe adjunctive treatment. We report a Japanese schizophrenic woman who was not in r...
متن کاملThe Treatment of Clozapine-Withdrawal Delirium with Electroconvulsive Therapy
Clozapine, a commonly used atypical antipsychotic, can precipitate a severe withdrawal syndrome. In this report, we describe a case of delirium with catatonic features emerging after the immediate cessation of clozapine subsequent to concerns of developing neuroleptic malignant syndrome. After multiple treatments were found to be inefficacious, electroconvulsive therapy (ECT) was initiated, res...
